Request and booking forms make it easy for clients to contact your business from your website, social profiles, ads, email signature, QR codes, and local listings. The goal is simple: wherever a client decides they need service, they should have a clear path to submit the request.

Use a request form when your team needs to review details before scheduling or pricing the work. This works well for estimates, inspections, custom jobs, urgent service, and work that needs photos or measurements.
Use a booking form when clients can choose a service and time directly. This works well for standardized services with clear availability.
Both forms can create records in Requests so your team can review new work in one place.
Copy a form link
From request and booking settings, copy the URL for the form you want to share.
Before sharing the link publicly:
- Open the form in a new browser tab.
- Submit a test request or booking.
- Confirm the submission appears in Requests.
- Confirm the client confirmation email uses the right wording.
- Confirm internal notifications go to the right users.
Use Email Templates to adjust confirmation messages.
Add the form to your website
Place the form link behind a clear call-to-action button.
Good button labels include:
- Request service.
- Book online.
- Schedule an estimate.
- Get a quote.
- Request a callback.
Add the button to high-intent pages, such as:
- Homepage.
- Contact page.
- Service pages.
- Pricing or packages page.
- Landing pages for ads.
- Thank-you pages.
- Footer or navigation.
Keep the button visible and easy to understand. Clients should not have to search for the next step.
Embed the form
If your website supports embedded forms, add the Fieldified form directly to the page so clients can submit without leaving your site.
After embedding, test on desktop and mobile. Confirm:
- The form fits the page width.
- Required fields are easy to complete.
- The submit button is visible.
- The confirmation message appears after submission.
- The request arrives in Fieldified.
If an embedded form feels crowded on mobile, use a button link instead.
Add the form to social media
Use the form link anywhere clients discover your business online.
Common placements include:
- Facebook page button.
- Instagram bio link.
- LinkedIn company profile.
- Local community posts.
- Paid ads.
- Link-in-bio tools.
- YouTube video descriptions.
Use short copy that tells the client what will happen after they click. For example: “Request service and our team will follow up with next steps.”
Add the form to Google Business Profile
Add the form link to your Google Business Profile when you want clients to request service directly from local search.
Use a request form for estimate-first workflows and a booking form for services clients can schedule directly. Test the link from the public profile after saving it.
Add the form to QR codes and printed materials
Form links also work well for offline marketing.
Use QR codes on:
- Door hangers.
- Flyers.
- Yard signs.
- Service leave-behinds.
- Business cards.
- Vehicle decals.
- Event materials.
After creating a QR code, scan it on a phone and submit a test request.
Add the form to email signatures
Add the form link to team email signatures so every reply gives clients a next step.
Examples:
- “Need another service? Request it here.”
- “Ready to book? Schedule online.”
- “Send photos and project details here.”
This is useful for repeat clients and leads who are still deciding.
Track and review submissions
When clients submit a form, review the request in Requests.
From there, your team can:
- Qualify the request.
- Schedule an assessment.
- Add notes or attachments.
- Convert the request to a quote.
- Convert the request to a job.
- Archive requests that do not move forward.
